This brand-new, interactive resource is brought to you in collaboration with the Peter Munk Cardiac Centre and Ajmera Transplant Centre! The Guide was created by a group of people with lived experience. This includes both patients and caregivers, as well as clinical advisors (Heart Failure Cardiologists, Heart Transplant Surgeons and Transplant Coordinators).
