This brand-new, interactive resource is brought to you in collaboration with the Peter Munk Cardiac Centre and Ajmera Transplant Centre! In time for Heart Failure Awareness Week, this resource is designed to support transplant patients and their loved ones at every stage of the journey—from initial assessment to surgery to thriving in life post-transplant. 

This engaging and easy-to-navigate webpage features expert insights, helpful tools, and, most importantly, real stories from people with lived experience. Through personal videos and interactive content, you’ll hear from individuals who have gone through the procedure themselves, sharing their challenges, triumphs, and advice for others on a similar path. 

Whether you’re preparing for a transplant yourself, supporting a loved one, or simply looking to learn more about the process, this resource is here to help. We invite you to explore, learn, and be inspired by the incredible community of transplant recipients and healthcare professionals dedicated to making the journey smoother for everyone. 

The Guide was created by a group of people with lived experience. This includes both patients and caregivers, as well as clinical advisors (Heart Failure Cardiologists, Heart Transplant Surgeons and Transplant Coordinators).
